About Us

Helping Hands Caring Souls is a Social Services based agency contracted in Milwaukee County to provide Supportive Independent Living Services and a State of Wisconsin Certified Medicaid Provider for Prenatal Care Coordination Services. Our agency is based on the belief that our customers' needs are of the utmost importance. Our entire team is committed to meeting those needs and making them a priority while maintaining professionalism and being knowledgeable of the members needs.  

Program Goals:

  • Encourage and train each member at his/her optimal pace, to maximize his/her functional skills for the purpose of achieving independent living and/or healthy pregnancy
  • Provide exercise to develop fitness, wellness, mobility and endurance
  • Facilitate enhanced independence through interpersonal skills training
  • Encourage increased participation in recreational, cultural and community events
  • Encourage the support of positive family involvement
  • Early and Continuous Prenatal Care Services
  • Connection to available community services
  • Assisting in accessing and obtaining needed health and social services
  • Assistance in locating job readiness programs when necessary

Population and Services Provided

Adults with Developmental Disabilities and/or Mental Health Diagnosis

Members in this population group will receive Supportive Independent Living/Supportive Apartment Services to assist them with becoming and remaining as independent as possible in the community while in their own home. Dependent upon the agreed level of care required for the Member, services will include but are not limited to, assistance and training with Daily Living Skills (ADLs), Meal Preparation, Medication Management, Coordination of Medical Appointments and assistance communicating with doctors, Coordination of Community Activities, and Shopping.

Pregnant Women receiving Medicaid and BadgerCare

Members in this population group will receive Prenatal are Coordination Services to assist them with having a healthy pregnancy. Services offered will include but are not limited to, the coordination of and connection to various community services such as Physicians, Counselors, financial advisors, medical and food assistance programs, Health education and nutrition counseling service, Care Plan Development, Ongoing Care Coordination and Monitoring, and employment programs if necessary.
